Offers case management, coordination of services, and comprehensive quality healthcare. Addresses factors which impact social determinants of health. The goals of the program are to:
- Increase access and utilization of health an social services (cross cutting)
- Improve preconceptions of health by decreasing risk factors for adverse life events among women
- Reduce infant morbidity and mortality
- Increase the proportion of adolescents and women of reproductive age who maintain a healthy weight
- Improve the cognitive, physical, and emotional development of all children
